Geriatric Care Management
 Geriatric Care Managers (GCM) consult with families, assess their loved ones needs, develop care plans and guide individuals and families to community services. A skilled GCM can be the key to maintaining independence for as long as possible by providing communication, advice, and support to family members caring for elderly or disabled family members. In addition to serving individuals and families, a Senior Solutions' Care Manager works with physicians, hospitals and community service providers to coordinate a continuum of services. Care, compassion, and concern, coupled with an extensive knowledge of community resources are the hallmarks of a great GCM.
